Medical Release form - Required for child to play in league. 169 KB, Adobe Acrobat (PDF). Please hold for your team's Manager.

Volunteer Application Form - Required for adults to participate as an assistant coach, coach, manager, team mother, or board member. 211 KB, Adobe Acrobat (PDF). Manager Letters are being accepted, and should be submitted to the league president as soon as possible.

Dates and Times

• Saturday, August 9^th from 9:00 AM to 12:00 PM
• Saturday, August 12^th from 6:00 PM to 8:00 PM
• Saturday, August 16^th from 9:00 AM to 12:00 PM

Register at our Driver Facility (click link for layout) in the Boardroom (located over concessions stand). Our registration fee is $50.00 per child. Family discounts for multiple children are available.
